In fact, it seems to stay on - after I started a new game and quit it (by pressing the BREAK key, that is, CAPS SHIFT and SPACE), it was still there as the new title screen. Yes, I think there's been a confusion in my wording - by "die" I meant "lose all lives". I don't think it's required to collect all items, just to reach the ending itself. jetsetdanny Posted August 6, 2023 Report Share Posted August 6, 2023 I've just tried it and I can confirm that it also happens when you enter the Central Cavern with 298 items collected (which is the minimum number of items necessary to trigger the toilet run).
